What we ask of you

At this time we will be open for appointments only, so please call ahead. This includes dispensing for eye wear or contact lenses.

Please fill out our online history form and update at home.

Please wear a mask to your appointment.
Any Patient with Fever, Cough, Malaise, or Contact with COVID-19+ Individuals:

Please do not come in if you have any symptoms of respiratory illness (fever, chills, cough, sore throat, muscle aches, malaise, shortness of breath, loss of smell or taste). Please call your primary care physician and seek medical care and guidance if symptoms are severe.
What you can expect from us

We have adjusted patient scheduling to reduce the number of patients in the office at any given time.
- All patients and staff will be masked as per current CDC guidelines.

We have plexiglass barriers at the optical dispensing tables.

All frames will be disinfected between patients, so you can enjoy your shopping!

All exam rooms, diagnostic equipment and the pretest area are disinfected before every patient encounter and common touch points in the office are disinfected regularly.

We now require intake forms to be submitted online in advance of your appointment to reduce the number of patients in the waiting room and to reduce your time spent in office.

Please let us know if you are having difficulty completing your forms online and we can do as much as possible via phone.

We are offering TeleMedicine including Virtual Consultations and Virtual Follow-Ups (for select appointment types) as options to reduce the number of patients seen in the office. Contact the office to learn more.
Our goals are to keep our patients and staff healthy and to deliver great eyecare. But we also want to help restart our community.
